Feedback Form for Volunteen Supervisors

    Does anyone have a feedback form for Volunteen Supervisors.  I would like to give them feedback on what they are doing that is great and areas for improvement.  Thanks, Michelle

    You want to give feedback to the Voluteen supervisors?  Or you want the supervisors to give you feedback about the teens?

    I want to give feedback to the supervisors. 

    Example:

    Good:

    Sue, your department is doing a great job supervising Sara, she loves the clerical duties she has been assigned. 

    Challenges:

    Sara bragged about how someone recently struck up a conversation about the movie "Twillight" with her.  The teens really enjoy interacting with staff and patients. 

    Something kind of like that is what I envisioned.  I have some departments that have a variety of task, keep the teens engaged and busy.  We love these departments,  then I have the ones that stand them at a scanner for 8 hours a day and have virtually no interaction with them.  (Those are the departments I really want to focus on to make these quality assignments next year.)  Your thoughts and suggestions are greatly appreciated!  Michelle

    This is a great idea!  However, I don't have anything like this at this time.

    I think I would I treat it as a Kudos.  As soon as I heard a positive comment from a teen, I would have Kudos from I could fill out and send to the department so the manager could hang it up for the whole department to see.  Hopefully this would encourage the staff members to continue the positive interaction.

    I handle the challenges as they come and usually with a phone call to the manager. ie. the volunteer is bored and does not feel like they have enough to do...do you have additional tasks that they could complete for you?  I find I can usually resolve any problems this way, but if a department continues to have problems, I will reassign the teen.

    I also have the teens fill out a survey at the end of their service asking how the department did and share the results with the managers.

    I recently completed a MASH program. It took the place of our traditional Junior Volunteer Program. I would like to survey the speakers and program planners that participated. The majority of these are hospital staff. Do you have any surveys for your staff that you would share with me?

    Our teens as well as parents complete an evaluation at the end of the summer program.  The department supervisor is evaluating the performance of the teen. 

    I then shared the teen and parents comments with the supervisor--minus any identification.  That gets the message across when the teen says I was bored and had nothing to do...or I felt like I was just another piece of office equipment.

    I used the supervisor evaluations to determine if the teen came back the following summer or could participate in the school year program.

    We have both our teens and the departments fill out suveys after the summer session is over.  We share with the departments a summary of the teen's comments.....good or bad.
